The Staff Funding Request form is available for Creekside High School staff to request funding. 
 

All requests are reviewed at the PTSO Board meeting, typically held on the first Friday of each month. Please allow up to 30 days for review and response.
 

Approved requests will be fulfilled in one of the following ways:

• A check issued to Creekside High School and managed by the school bookkeeper, Crystal Stone, or

• Direct purchase of requested items by the PTSO, with delivery to the school
